    ​                                   CONSENT TO PLAY, WAIVER AND LIABILITY OF RELEASE FOR 2019 CLASH CAMP
    I hereby give permission for my child, whose information has been listed above, to     participate in the 2019 Clash Camp being held in the town of Kennebunk, Maine on 7/8/2019 through 7/12/2019. I am aware and acknowledge that soccer is a contact sport and that soccer competition carries with it a potential for serious injury. I am aware of no medical conditions, impairments, restrictions, illnesses or injuries that would prevent or limit my child from participating in all aspects of the camp, and any medical conditions that do exist (allergies, medications currently being taken, etc.) are described with my child’s registration form.

    I understand that the Kennebunk Soccer Club, Cavanaugh Coastal Coaching LLC and their representatives reserve the right to dismiss participants who violate any town, school district rules stated or implied, or whose behavior or style of play is considered unsportsmanlike, uncontrollable, or at risk to other players.

    I hereby give permission to medical personnel to provide emergency medical assistance and hospitalization, if necessary, to me / my child in case of accident or injury. I understand that there is a risk of injury to my child as a result of his/her participation in the camp and I knowingly, voluntarily and fully assume all risk of such injury from such participation, even if arising from the negligence of the participants or others. I agree to indemnify and hold harmless the Kennebunk Soccer Club, Cavanaugh Coastal Coaching LLC and any of their employees, volunteers, referees, or coaches in any capacity for any and all injuries, damages, causes of actions or claims arising from my / my child's participation in this camp, including, but not limited to, for personal injuries or property damage.
